Can a Chiropractor Help With a Herniated Disc? A Guide for Patients in Gqeberha

Being told you have a herniated or bulging disc can be frightening.

For many people, the first thought is that something in their spine has slipped out of place or that they've permanently damaged their back.

Then come the questions.

Will I need surgery?

Is it safe to exercise?

Should I avoid bending?

Can a chiropractor help?

These are questions I regularly discuss with patients experiencing back pain, sciatica and other symptoms associated with spinal disc problems.

And one of the first things I try to explain is this:

A disc herniation doesn't automatically mean your spine is permanently damaged or that surgery is inevitable.

Many people improve with appropriate conservative care.

The important thing is understanding what the diagnosis means, how it relates to your symptoms and which treatment options are appropriate for your particular situation.

What Is a Herniated Disc?

Between the vertebrae of your spine are structures called intervertebral discs.

These discs help distribute loads and allow movement between the bones of your spine.

Each disc has a tougher outer layer surrounding a softer, gel-like centre.

A disc herniation occurs when part of the inner disc material pushes through a weakened or damaged area of the outer layer.

A bulging disc is slightly different. It generally describes a broader extension of the disc beyond its usual boundaries.

Both terms describe structural changes in the disc, but they don't automatically tell us how much pain someone should experience.

Some people have disc bulges or herniations visible on an MRI without experiencing significant symptoms.

Others experience considerable pain, particularly when nearby nerve roots become irritated or compressed.

The MRI finding is important, but it needs to be interpreted alongside the patient's symptoms and examination findings.

Does a Herniated Disc Always Cause Back Pain?

No.

This is one of the most important things to understand about disc problems.

Disc changes are relatively common, particularly as we get older.

And not every disc abnormality found on imaging is responsible for someone's symptoms.

A person may have an MRI showing a disc bulge at L4/L5, for example, but their pain may not correspond with that finding.

That's why I don't believe in treating an MRI report in isolation.

I want to understand the person sitting in front of me.

Where is the pain?

Does it travel down the leg?

Is there numbness or tingling?

Are there signs of muscle weakness?

What movements aggravate the symptoms?

What does the neurological examination show?

The answers help determine whether the disc finding is clinically relevant.

Why Disc Problems Sometimes Develop Gradually

Many patients can remember the exact moment their back became painful.

They bent down to pick something up.

They lifted a suitcase.

They turned awkwardly getting out of the car.

Suddenly, they experienced severe pain.

Naturally, they assume that movement caused the entire problem.

Sometimes an injury really does happen suddenly.

But disc changes can also develop over time through a combination of ageing, genetics, previous injuries and mechanical loading.

The painful episode may be the first time someone becomes aware of a problem that has been developing gradually.

That doesn't mean poor posture or incorrect movement necessarily caused the herniation.

And it certainly doesn't mean the patient should blame themselves for bending or lifting.

**Our spines are designed to move and tolerate load. The goal is to help them regain that ability, not make
